Tainan City Government have implemented the Influenza Electronic System (CIVS) to provide the "Campus Influenza Vaccine Consent Form" online for students in our city. This consent form primarily seeks to inquire whether you agree to have your child receive a vaccine. In accordance with this policy, our school will no longer provide paper forms for parental signatures. Please follow the instructions below to sign the online vaccine consent form:

Regardless of whether you intend for your child to receive the influenza vaccine, you must complete the online signing during Oct. 16th to Oct. 31st 23:59.
You can modify your consent online at any time before the vaccine is administered.
Our school plans to administer the influenza vaccine for G7 to G12 students on Nov. 15th (Wednesday).

Since the system provided by the Ministry of Health is currently available only in Chinese, and we have also checked with the Ministry of Health, there will not be an English version in the near future. I have found a video on YouTube; you can watch this video for further instructions. Please also check Vaccination Guidelines before you sign your name.
