Dear IBST parents, To cooperate with the Education Bureau's investigation, please fill out this form if your child is willing to receive the COVID-19 XBB.1.5 vaccine on campus, and please provide relevant information for registration to facilitate arrangements by the administering medical unit.

• Currently, there is no confirmed vaccination schedule. The school health center assists in surveying willingness to receive the vaccine, and subsequent arrangements will be made by the local health bureau. Therefore, even after the announcement of the confirmed vaccination schedule, willingness can still be cancelled.
• Those willing should fill out the form, while those unwilling can skip filling it out.
• The vaccine to be administered this time is the Moderna (Spikevax) XBB.1.5 COVID-19 vaccine.
