Dear Parents,

The Tainan City Government has implemented an electronic vaccine system (NIAS) starting in 2024, providing the "Flu Vaccination Consent Form" and "COVID-19 Vaccination Consent Form" for students in our city. Please refer to the following instructions:

1. Parents, please scan the exclusive QR code(attached below) for “ National Nanke International Experimental High School (including the attached Junior High and Elementary School)” using your mobile phone to access the consent form. You can complete the signing process online. (Please ensure to sign with clear and legible handwriting. Thank you.)
2. For parents of G7-G12 students, regardless of whether you wish for your child to receive the flu vaccine and/or COVID-19 vaccine, you must complete the online signing process by October 1 (Tuesday) at 11:59 PM to facilitate the counting of numbers. G7-G12 students are scheduled to receive the flu vaccine on October 24 (Thursday) and the COVID-19 vaccine on November 14 (Thursday).
3. For parents of G1-G6 students, regardless of whether you wish for your child to receive the flu vaccine and/or COVID-19 vaccine, you must complete the online signing process by October 7 (Monday) at 11:00 PM to facilitate the counting of numbers. G1-G6 students are scheduled to receive the flu vaccine on October 24 (Thursday) and the COVID-19 vaccine on November 7 (Thursday).
4. Consent can be modified before the vaccination day.

   Please note that the two links above represent the flu and COVID-19 vaccines respectively, and must be filled out separately.
6. Please make sure to bring your National Health Insurance card on the day of vaccination.
