Dear Parents,

The 115th Academic Year (2026) Student Influenza Vaccination Program is conducting its survey using an "Online Vaccination Consent Form." Please read the instructions below and complete the consent form and digital signature (Please sign clearly) by the deadline.

2. Important Reminder: Whether you consent or decline your child to have    the flu vaccine, please complete the online consent form by 11:59 PM on Monday, October 5th.
3. If the student is under 18, the consent form must be signed by a legal guardian. If the student is 18 years of age or older at the time of signing, they may sign the form themselves.
4. If you wish to change your response after submission, simply log in to the system again to make modifications.
5. Students must bring their Health Insurance card(健保卡) on the day of vaccination!
